Please complete the following form using the same email address you used for registration. The form must be complete BEFORE the first game/meet of the season for your child to participate in games/meets. If you have multiple children participating, a separate form must be filled out for each child. This form is only required one time per school year per child. If your information has changed from season to season, please fill out the form again.
